Health Technology Assessment, the Past and the Future

Abstract
Increasing number of countries in the world adopt the health technology assessment (HTA) system for allocation of scarce health resource. Asia region is no exception to this transition toward HTA environment. HTA in Asia could be said singular in two respects, 1) Asia is a highly populated region with huge size of potential customers of new health technologies, and 2) Asian economy on average is growing faster than any other regions in the globe, making the region an attractive future market for health technology industry. It is interesting to note that Korea has become a leader in terms of HTA system development in Asia. Along with this emerging trend in Asia, naturally developed are a lot of research interests among academic researchers and stakeholders, where stakeholders encompassing decision-makers, industry researchers, patients, and health practitioners. A challenge accompanied by is the consistently evolving conflicts of interests among stakeholders, as different stakeholders possess different views, perspectives and objectives. One could also observe that the subject HTA is evolving very fast globally as new frontiers are added on to the existing ones. Major evolutions take place in its methodology as a science of evidence generation, and in policy framework as a linkage between generated evidence and decision making for resource allocation. Examples of changes in the methodology and policy areas include; HTA for personalized medicine, conjoint analysis, mixed treatment comparison, use of real world data, coverage with evidence development, risk sharing scheme, and structured budget impact analysis. System of HTA in principle is pursuing ¡®technical efficiency¡¯ in resource allocation. Its ultimate objective is to achieve Pareto Efficiency. In its policy framework, ¡®utilitarianism¡¯ is the underlying social philosophy. Therefore, in any system of HTA, including that of Korea, the notion of Pareto Efficiency and utilitarianism has to be embedded in one way or another.
